ONC announces Dr. Antonio Disclafani’s retirement
We are regretful yet joyful to announce Dr. Antonio Disclafani’s retirement from the practice of neurosurgery. Dr. Disclafani has worked as a neurosurgeon for 30+ years. Although he will greatly be missed at our office, we wish him much luck in whichever future endeavors he pursues. He has added much value to our office and has our unending support. Below is a letter Dr. Disclafani has written for you:
I am writing this letter to inform you that I will no longer be able to serve as your neurosurgeon at Ocala Neurosurgical Center in Ocala, FL.
After 30+ years of an enjoyable and fulfilling career, I have decided to retire from the practice of neurosurgery.
I will continue to see my patients with follow up appointments in the office until October 29, 2019. Due to follow up care guidelines I will not be performing any new surgical cases.
Of course, you may choose to continue your care in the same office with Dr. Mark Oliver, Dr. Daniel Robertson or Dr. Jacob Freeman where your patient records will be kept, and you will continue to receive excellent care. If you choose to see a different provider, please go to our website www.ocalaneurosurgeons.com where you will find medical records request forms for you to print or you can have your records requested by your new physician.
If you choose to continue to be seen by Ocala Neurosurgical Center, no action by you is required. Thank you for trusting me with your needs throughout my time at Ocala Neurosurgical Center. It has been a pleasure caring for you. I wish you continued health and happiness in the future.
